As soon as you join Chronius, and schedule your first appointment, you’ll get access to the Chronius Care Empowerment Platform. 

Prior to your first visit, our team will use this platform to pull together your historical medical records and identify the current state of your health. Your care team will pour over the details, so we’re up to speed on your journey from Day 1.

You’ll also have instant access to the platform’s symptom tracker, medical education, and more!

Meet your Care Team

Our ComprehensiveCare Kickoff is an extensive initial meeting with your care team. You’ll get paired with a physician, nurse practitioner, and care advocate who will delve into your health history, explore your current symptoms, and set your long- and short-term health goals, over a video call.

Get your Personalized Care Plan

Together, your care team will create a holistic care plan to tackle your short-term goals, like finding a diagnosis or building out a specialist team you can trust, and long-term health goals (hello improved quality of life and under control symptoms).

This plan may include testing, medication, dietary and lifestyle changes, counseling, and adding specialists to your team. Our number one goal is to help get you feeling better — and we’re here by your side no matter what gets thrown your way.

Access In-Depth Follow Up Care

Your care team will provide ongoing video visits and messaging support to manage symptoms, stay on-track with your health goals, and adjust your care plan and medication as needed.

We provide hands-on care, even while virtual, by using remote patient monitoring tools and partnering with providers that can, if needed, offer physical exams, administer IVs, and more.
