Dr. Kate Eisenberg

Primary Care Physician

  • Kate Eisenberg, MD PhD has ten years of experience in delivering primary care in addition to her training in epidemiology and board certifications in Family Medicine and Clinical Informatics. She partners with her patients to support evidence-based care decisions, taking into account patient values and preferences.

  • I view my role in primary care as informing, supporting, and advocating for patients as they navigate their medical conditions, choices, and the healthcare system. I feel it is critical for patients to have as much high quality information as possible given how complex medical care can be today.

  • I have seen in primary care how difficult it can be for patients to navigate the healthcare system. I find it fulfilling to support patients in understanding the healthcare system and their medical conditions so they can make decisions about their care that make sense to them and that they feel confident about.

  • I’m chronically experimenting to try to bake the perfect chocolate chip cookie. It’s always fun even if they don’t come out perfectly.

Dr. Jen Rubin

Primary Care Physician

  • Jen Rubin, MD, is a double board-certified internist and pediatrician. She earned her MD in 2007 from Georgetown University School of Medicine. While there, she was an honored recipient of the Michael J. Caruso Memorial Award for Humanitarian Principles in Patient Care, and was inducted into the Alpha Omega Alpha national medical honor society. Jen subsequently completed her residency in both Internal Medicine and Pediatrics in 2011 at Yale New Haven Hospital. She spent the next 12 years practicing for a large healthcare system in Connecticut. Jen has a special interest in caring for patients with chronic medical conditions. She views the practice of medicine both an honor and privilege, and is known for her tireless dedication to caring for, educating and advocating for her patients.

  • Every patient has a unique story with individual challenges, but a uniform need for top quality medical care. The ability to provide this care relies on a strong knowledge base, broad clinical experience, empathy and trust. I am ready to listen, understand and help, and am dedicated to meeting patients where they are and tailoring plans to get them where they want to be.

  • Chronic medical illnesses are more common than most people realize. Patients’ journeys are filled with unexpected twists and turns from diagnosis to management to merely navigating the day-to-day. The physical and emotional impact is significant, making the ability to accurately diagnose and manage their conditions uniquely beneficial and rewarding. The opportunity to be part of the Chronius Team, a team that understands the complexities of chronic medical conditions and is determined to help navigate the journey, is so exciting.

  • I’m chronically fascinated by cultural variations in food, not only type and flavor, but also cost and accessibility. This, combined with my husband’s contagious love for travel, has led me to pay visit to a grocery store in every country to which I have been fortunate to travel. While most people are excited to visit sites in these destinations, I cannot wait to see their grocery stores!

Dr. Callie Pederson

Primary Care Physician

  • Callie Pederson, DO, is a board-certified Family Medicine physician with 7 years of clinical experience. After graduating from the Alabama College of Osteopathic Medicine, Dr. Pederson began her career as a physician in the United States Air Force, and served as Chief of Medical Staff for the 28th Medical Group for 2 years. During this time she led efforts to improve care quality, reduce risks, and advocate for patients. Dr. Pederson is passionate about providing quality, patient-centered care, especially for those with complex medical needs.

  • I use an evidence-based approach that involves shared decision-making. It's important to me that patients are well-informed about their health and play a central role in making decisions. As a primary care provider, I also see myself as an advocate, helping patients navigate the healthcare system and coordinate care with specialists.

  • I am thrilled to be a part of Chronius where I get to be part of a clinical team dedicated to making every patient's experience the best it can be! After recently managing my own chronic illness, I was excited to find Chronius, whose mission strongly resonates with my new clinical focus.

Jennifer Lievano

Nurse Practitioner

  • With over 20 years of experience in internal medicine, family medicine, and women’s healthcare, Jennifer has dedicated her career to making a difference in the lives of my patients and their families. She started her career as a registered nurse in 1993 after graduating from the University of Central Florida. Later, Jennifer continued her education at Florida Atlantic University in Boca Raton. She is board certified in Family Practice and provides care for patients with compassion, dedication, and advocacy.

  • Patients are the experts of their own experience. I provide medically-backed, personalized guidance that helps patients take control of their journey from symptom tracking to treatment decisions. It is my mission to serve those with chronic and rare diseases while listening and making them feel heard.

  • I was drawn to Chronius because it reimagines chronic care with the clarity, coordination, and compassion that patients with rare and complex conditions deserve. I understand this because I have experienced the struggle/heartache to find and provide care for my own child with a chronic disease. It is important for me to serve this population.

  • When I am not delivering care to my patients, I enjoy traveling the world. Cruising is one of my favorite ways to vacation. I love to experience diverse cultures and sample different cuisine from various regions around the globe. I also love a good Pilates class!

Patricia Martin

Physical Therapist

  • Trish Martin, PT, DPT, is a highly experienced physical therapist and committed advocate for individuals living with chronic illness. Motivated by her daughter’s diagnoses of EDS and POTS in 2020, Trish immersed herself in research, attended conferences, and collaborated with leading experts to better understand how to manage these complex invisible conditions. Today, she continues to raise awareness, educate healthcare professionals, and promote nationwide collaboration to improve care. In recognition of her impactful contributions, Trish received the Orlando Health Allied Health Excellence Award in 2023.

  • I believe in meeting each person exactly where they are. No two chronic illness journeys are the same, which is why I take the time to understand how your conditions impact your daily life and what goals matter most to you. Together, we create a personalized plan to move you forward—one meaningful, manageable step at a time.

  • I am truly inspired by Chronius’s innovative approach to treating chronic conditions—blending available technology with deeply compassionate, patient-centered care. I believe this unique integration will change the health care paradigm and move medicine in the right direction. I’m honored to be part of the journey.

Dr. Karim Hanna

Practice Director

  • Karim Hanna, MD, is an academic physician board certified in Family Medicine and Clinical Informatics. He has served in many roles at the USF Morsani College of Medicine, most recently he is the family medicine residency program director. He is a dedicated physician and chief of Family Medicine at Tampa General Hospital. He has a keen interest in health tech, using data, AI and technology to improve patient outcomes and change how we teach.

  • His approach to care is holistic and patient-centered, reminding patients that they are in charge of their care. He focuses on the entire family unit and regularly reminds himself and his students to love the patients.

  • I am thrilled to contribute to Chronius' mission because it aligns perfectly with my own goals of improving the management of chronic disease. I want to be proactive, assisting patients before their problems are overwhelming. We should not wait till the disease process has progressed without equipping our patients. Chronius will be a game-changer.

  • I am a husband and a father first. When I am not with them, I’m involved with my church community. I am chronically training and exercising and love reading or relaxing on a vacation.

Dr. Leah Probst

Primary Care Physician

  • Dr. Leah Probst is a board-certified Internal Medicine Physician who earned her Doctor of Osteopathic Medicine (D.O.) from Des Moines University. Dr. Probst completed residency training through the Aurora Internal Medicine Residency Program in Milwaukee and has 7 years of primary care practice, all with Advanced Aurora Health Care.

  • I prioritize a personalized model of care, recognizing that every patient has a distinct story and set of needs. I specialize in creating care plans that honor the individual goals while providing the consistent, compassionate support necessary to navigate a lifelong health journey.

  • I joined Chronius because I am passionate about helping people achieve their best emotional and physical health. I love working with a caring team that does an amazing job supporting patients in a collaborative environment.

  • I love gardening and am chronically looking for ways to add more plants to both my yard and my house!  Green multiplies everywhere I go.

Casey Berna, LCSWA

Mental Health Therapist

  • Casey Berna*, LCSWA, received her Master’s in Social Work from Fordham University over twenty years ago. Her own challenges navigating co-occurring illnesses and chronic pain inspired her to work with patients in these communities. Casey has a passion for shining a light on the intersections of chronic illness, reproductive health, and mental health. She is a patient advocate in the endometriosis, infertility and pregnancy loss communities and collaborates with many nonprofits in these fields to improve access to care.

  • Having a chronic illness and chronic pain can have a huge impact on mental health and often can create feelings of depression and anxiety. I help patients process any grief or medical trauma that they have as well as work to reduce stress, create boundaries, and navigate relationships in their life that may be challenging. Sometimes a session can simply be a place to feel heard and validated.

  • I know firsthand how living with a chronic illness and chronic pain not only has an impact physically, but impacts every single aspect of life. Chronius recognizes this and is committed to help reduce the stress and burden that patients experience through a multidisciplinary approach.

  • As someone who loves Italian food and baked goods, I am chronically on the lookout for gluten free items and recipe hacks that taste as good as the gluten filled ones and am happy to share my favorites!

Sara Graff, LCSW

Mental Health Therapist

  • Sara Graff*, LCSW is a licensed therapist. She has been in private practice for over 15 years and has experience providing therapy across the age spectrum. After obtaining her Master in Social Work degree from Washington University in St. Louis, Sara has worked in a variety of settings including hospitals, physician offices, community mental health centers, and private practice. Currently, Sara’s practice focuses on helping people move through the anxiety that comes with life transitions such as chronic illness, aging, relationship issues, caregiver stress, or career problems.

  • I focus my therapy on combining cognitive behavior therapy (CBT), mindful based interventions, and positive psychology. I guide you down a path to learn how to hope, peace, and acceptance in your life. In our sessions, we learn how to challenge fearful thoughts, live in the present moment, and practice gratitude.

  • I understand what it is like when you feel like a hurricane has just hit you when you receive that diagnosis. I understand personally the challenges facing you when life throws that curve ball at you and the unexpected arises. I believe in the strength of a strong support team who communicates and helps you move through your treatment towards a better quality of life.

  • I chronically photograph sunsets. I love sunsets and the beauty within them. My phone is filled with pictures of sunsets of all kinds of colors!

Jennifer Shapiro, RDN

Registered Dietician

  • Jennifer Shapiro*, RDN is a Registered Dietitian-Nutritionist with a master’s degree in nutrition and exercise science. Jennifer has a broad range of experience providing nutrition therapy in hospitals, outpatient settings, and private counseling. Jennifer is passionate about connecting with people and helping clients overcome their obstacles to optimize their health and wellbeing.

  • My goal is to provide patients with evidence based nutrition education as well as empathetic counseling to address the physical, mental and emotional aspects of their health journeys. I focus on helping patients foster a healthy relationship with food as well as making long term behavior changes to enable patients to take control of their health.

  • Dealing with a chronic health condition takes a village, which includes a supportive and collaborative clinical team. I’m excited to be a part of this clinical “village” to give patients the comprehensive care they deserve.

  • I chronically go to the beach to build sand castles with my daughter!

Chris Van Haren, BCPA

Care Advocate

  • Christine Van Haren, BCPA, RPh, has a lifetime of experience helping people navigate the healthcare system - as a parent, a Pharmacist, and a Board Certified Patient Advocate. She graduated from the University of Wisconsin - Madison School of Pharmacy and has worked in both community and long term care settings. Not content to watch her patients continue getting lost in our broken system, she returned to the UW - Madison campus to attend the Center for Patient Partnerships and hone her advocacy skills. Having obtained national certification as a BCPA in 2018, she now devotes her time entirely to Patient Advocacy and mentoring other Advocates.

  • As a Patient Advocate my role is to provide my clients with the tools and resources to make informed decisions about their health care and obtain the right care for THEM. My goal is to equip and empower my clients so they can live their best life - physically, mentally, emotionally.

  • I am thrilled that Chronius is so committed to its mission to reduce the burden of living with chronic illness that they offer on-demand Advocacy services for their members. It is a privilege to be part of the Chronius team.

  • I have come to accept the fact that I am a “chronic book buyer”. I love a good old fashioned book store, have an ever-changing stack of books I have yet to read, and will never apologize for buying more. So many books; so little time!

Samantha Weidner, BSN, RN, CCM

Head of Care Coordination

  • Samantha Weidner is a seasoned nurse leader with over 15 years of experience in care management, population health, and value-based care. She has built her career around improving outcomes for individuals with complex and chronic health needs through education, advocacy, and data-informed care. Known for bridging clinical expertise with compassion, Samantha has led multidisciplinary teams in both startup and enterprise healthcare settings while always focused on making healthcare more personal, accessible, and effective.

  • My care approach is built on the belief that meaningful outcomes arise when patients and their unique experiences are at the center. Listening deeply, empowering informed choices, and honoring each individual's needs guides every plan and collaboration. Compassion and data-driven insight together shape care that meets people where they are and adapts as life unfolds.

  • I joined Chronius because the mission deeply reflects my own - to reimagine chronic care in a way that restores connection, understanding, and trust between patients and their care teams. Having navigated and living with a chronic condition, Ehlers-Danlos Syndrome, I know firsthand how powerful it can be when patients feel truly seen, supported, and guided through their health challenges. 

  • I'm chronically exploring new ways to blend wellness and creativity - whether it’s designing fun healthy recipes, finding new movements in the gym, or exploring the wilderness and new experiences with my husband and our son.

Kristy Mize, LPN

Nurse Coordinator

  • Kristy Mize, LPN, is a military-trained LPN with 20+ years of experience across diverse nursing roles, including hands-on care in many specialties, care coordination, prior authorization, utilization review, and remote nursing. Skilled in managing chronic conditions and streamlining care, with a strong commitment to patient advocacy and outcome-driven care.

  • My approach to chronic illness care is centered around building strong, supportive relationships with patients while helping them navigate a complex healthcare system. I focus on understanding each patient’s unique needs, coordinating between providers, managing medications, and ensuring timely authorizations and follow-ups. By combining clinical knowledge with insurance and remote care experience, I work to reduce barriers, improve communication, and empower patients to take an active role in their health.

  • As both a nurse and someone living with a chronic condition, I know firsthand how overwhelming the healthcare system can be. With over 20 years of experience in nursing, care coordination, and insurance navigation, I joined Chronius to help bridge those gaps—bringing compassion, clarity, and support to patients who need it most.

  • I'm chronically found with a book in one hand and a whisk in the other—I’m an avid reader and a passionate baker who firmly believes every good story pairs well with something sweet from the oven.

Faith Smith

Referral Coordinator

  • Faith brings years of healthcare experience, including roles in phlebotomy and eye care, as well as a consistent passion for supporting people through meaningful care. Known for her attention to detail, she focuses on getting the “little things” right, which ensures each referral is accurate, organized, and aligned with the patient’s needs and goals.

  • I believe care should feel like a partnership. That means I take time to listen, explain things in everyday language, and make sure you never feel like just another number. Whether it’s vetting offices for your referral or just being someone you can reach out to, I’m here to help lighten the load.

  • Chronius stands out to me because it puts people first. We’re not just managing care—we’re building relationships and restoring confidence. As someone living with chronic health conditions myself, I know how exhausting and disheartening the traditional healthcare system can be—especially when your needs don’t fit neatly into a box. What touches me most about Chronius is our mission to change that. We create a space where patients with complex, ongoing challenges are met with patience, kindness, and support. I joined because I believe everyone deserves to feel seen, heard, and cared for—and at Chronius, that’s exactly what we offer.

Melissa McLearran, LPN

Lead Nurse Coordinator

  • Melissa McLearran, LPN, has more than 14 years of multifaceted nursing experience spanning primary care, geriatrics, and multiple specialty areas. Her clinical background is strongly rooted in advanced care coordination and comprehensive case management, with a consistent focus on supporting patients through complex health needs.

  • I believe in advocating tirelessly for my patients by providing the tools, education, and resources necessary to navigate an increasingly challenging healthcare landscape. My goal is always to make each patient’s care journey as seamless and supported as possible.

  • Working with Chronius empowers me to create meaningful impact in the lives of patients facing chronic and complex conditions. My passion for this work is deeply personal—shaped by witnessing my mother’s journey with lupus and other chronic illnesses, as well as navigating my own ongoing health challenges. Chronius allows me to serve patients in a way that aligns with my values, experience, and commitment to whole-person care.

  • I’m chronically in pursuit of travel and adventure. I love spending time outdoors, exploring new destinations, and experiencing different cultures. Now that my children are grown, my husband and I take every opportunity to travel—by road, air, or sea—whenever we can.

Kim Schwartz, LPN

Nurse Coordinator

  • Kim Schwartz is a Licensed Practical Nurse with over 13 years of experience in healthcare, specializing in chronic care management, care coordination, and patient support. Her background includes working in primary care, rehabilitation, assisted living, pain management and admissions, giving her a well-rounded understanding of both clinical care and the systems that support it.

  • I believe in meeting patients where they are and creating a supportive, judgment-free space where they feel heard and understood. Managing chronic conditions can feel overwhelming, so I focus on breaking things down into manageable steps while ensuring patients feel supported every step of the way.

  • I was drawn to Chronius because of its commitment to truly supporting patients beyond traditional care. I value the proactive patient-centered approach and the focus on helping individuals navigate the complexities of chronic conditions in a more personalized and meaningful way. It’s exciting to be part of a team that prioritizes both quality care and genuine connection.
